Thank you for your interest in attending and completing the National Pool Lifeguard Qualification with Active Leeds. Please use this course prerequisite check to ensure you meet all the required criteria before completing your booking.
It is important you answer these questions to the best of your knowledge to ensure the correct advice is provided
I understand that if I complete the booking process and I am unable to meet the criteria, on the first day of the course, I may be unable to continue with the course and will forfeit the course fee.
Should you wish to complete a swim test with member of the team prior to booking then please contact leeds.lifesaving@leeds.gov.uk and then process your booking once this is complete.

The information you provide on this form will only be used in order to administer your booking for the National Pool Lifeguarding Qualification with Active Leeds. The council is the data controller of the information you provide and we are processing this data, contractually, in order to provide this service to the candidate.
The personal information you provide will be held for a period of 12 months following your last known contact with the service, after which it will be securely destroyed.
