Introduction

Leeds City Council is launching an Active Travel Social Prescribing pilot scheme in Burmantofts, Harehills and Richmond Hill. The scheme will run from October 2022 to October 2025  and is funded by the Department for Transport (DfT).

The purpose of the Active Travel scheme is to encourage more people to take more journeys by walking, wheeling, or cycling and is being offered by healthcare professionals as part of this pilot to improve people’s mental and physical health and wellbeing.

By taking part in the pilot scheme, you will be asked for personal and special category information

 

Your data will be collected at the following points

•             This consent form

•             Referral from Active Leeds

•             Referral from Burmantoft, Harehills, and Richmond Hill pathway

•             Self-Referral  

•             SmartSurvey Consent Form

•             DfT Survey (Beginning of referral, six months after referral, and twelve months after )

•             Interviews

•             Focus Group/Workshop

•             Community events

 

Obtaining consent 

We require your consent to collect and process the following personal information: First name, postcode, Partial Postcode (e.g. LS12), telephone number, email address, Gender, Date of referral, age range, date of birth, employment status, annual income, referred activity, and Unique Reference Number (URN).

We require your explicit consent to collect and process the following special category data: ethnicity, mental health, physical health and sexual orientation.

Who will be handling your data and how

Leeds City Council are the data controllers for the pilot scheme. The data processor is Urban Foresight Limited. Active Leeds will share your Unique Reference Number (URN) with the processor, Urban Foresight Limited who additionally will collect and process both personal and special category data during the pilot and produce a report for Leeds City Council and the Department for Transport. This report when published will contain pseudonymised data and document the effectiveness of the Active Travel Social Prescribing pilot scheme.

 

Active Leeds and Urban Foresight Limited will be able to identify you from the URN and any personal data you provide, they will use your data to keep in touch with you during the pilot scheme. However, you and your data will not be identifiable to the Department for Transport for the final report as the information will be aggregated and pseudonymised.

 

In order to evidence your consent to taking part in this pilot scheme and contact you, Leeds City Council will use Smart Survey to capture the following personal information: Name, Contact Number, and Address. Smart Survey are currently contracted by LCC. The personal and or special category data you provide during the pilot will strictly be used by the pilot and no other scheme. Your data provided to LCC, Urban Foresight Limited, Department for Transport and Smart Survey will remain in the UK and not leave the country. Your data will be held for three years after completion of the pilot scheme in October 2026. All personal and special category data will be destroyed after three years from the pilot end date.

 

Your right to withdraw your consent.

As outlined in UK GDPR Legislation, you have the right to withdraw consent at any time. Should you no longer wish for your information to be processed as part of the Active Travel Social Prescribing Pilot Scheme, please contact DfTreferrals@Leeds.gov.uk
