Relative and Friend Questionnaire

Introduction

Everyone has the right to expect high quality support. Thanks to the people who have taken the time to tell us about their experiences, we can and have improved the services we provide.

This questionnaire asks about your experiences in five key areas and will help us continue to ensure your relative or friend, and others like them get the quality care and support they need. It will take you about 5 minutes to complete and we will ask you:

• Whether the service is safe
• Whether the service is effective
• Whether the service is caring
• Whether the service is responsive to your needs
• Whether the service is well led

Each question is scored from very satisfied to very dissatisfied.
 
The results of these questionnaires will be considered carefully and improvements made in light of the feedback we receive.
