School Food Friendly Parent Survey

This survey is being run by the Health and Wellbeing Service at Leeds City Council
Dear parent,

Your child’s school has recently self-validated for School Food Friendly Status. As part of this process, the school will shortly be externally-validated by the Health and Wellbeing Service and we would like to involve parents. Your opinion does matter and we would welcome your thoughts on the following questions.

It should take you less than 10 minutes to answer all the questions and all responses are anonymous.

If you have more than one child at this school, and feel that your responses for each child are different, you can complete the survey more than once.

If you need to speak to someone about this survey, please speak to the Healthy Eating Lead in your child’s school.
